Hair transplant is a very personalized procedure. Patients can decide on many aspects such as graft amount, transfer areas and most importantly the hairline. Determining the shape and height of the hairline is a crucial step to achieving natural results. Patients don’t have to go back to their old hairline shape, they can pick out a new one for the transplant. A few factors go into deciding the most suitable hairline shape for each patient. We will cover these factors and the details in this article.

1- Effects of Aging
Aging changes the metabolism and the appearance of the body in many ways, one of which is hair loss. Hair loss is not always symmetrical or linear, which causes the hairline to start changing. The two ways in which hairline can change are “receding hairline” and “maturing hairline” processes. Receding refers to the shape of the hairline changing due to hair loss on the front parts of your hair. Maturing means the hairline going higher and becoming less defined. Hairline maturing can happen as early as mid-twenties. While maturing can happen to everyone, receding is usually seen in men as it’s caused by male pattern baldness.
Transferring hair to the upper forehead area can lower the hairline and combat the effects of aging. The length between brows and hairline is usually different for men and women because men have longer foreheads. This length is 6 to 9 cm (2.4 – 3.2 in) for men and 5 to 6 cm (2 – 2.4 in) for women.

2- Facial Shape for Women
Face shape is the largest contributing factor to choosing a hairline. Although there should be no “ideal” facial shape, there are very flattering looks you can achieve with the correct hairline. The most common facial shapes are oval, square, heart and round. The main idea behind choosing the most suitable hairline for each patient is considering their jaw, cheeks and forehead.
Patients with strong, wide jaws or round faces could opt for an inverse-U shaped hairline. This option would result in a longer looking face. It would also define the face and give it a more structured appearance.
Patients with narrow or pointy chins can go for a heart-shape by choosing an inverse-V shape. This creates a small triangle in the middle of the hairline. Otherwise, they could also choose an inverse-U shape which would achieve a more oval look.
3- Facial Shape for Men
For male patients, the most attractive facial shape is “square”. The square face shape highlights the strong jaw and brings forth a high forehead.
An important thing to note is that the straighter the hairline is, the more attention it brings to itself and the forehead. Thus, if patients like their jaw and chin, they can choose from a variety of hairlines. This can be a heart-shape, inverted-U or inverted-V. Patients that want to bring attention away from the forehead should opt for more structured and straight hairlines. This would draw attention to the forehead rather than the chin.
For example, if a patient has wide cheeks and a narrow jaw (round facial type), they would benefit from a straight hairline. They can also go for a low-angle inverted-V shape, where the hair around the temples are further back than the center. Another example would be a patient with a wide jaw and defined face. As they have the “ideal” facial type, they can go for whichever hairline they would like. Although a straight hairline would also be the most suitable, as it would make the jaw more prominent.
4- Effects of Ethnicity
It’s also important to consider the ethnicity of patients as it plays a major role in determining the hairline. People with African or Middle Eastern descent usually have straighter and thicker hairlines. In turn, people with Caucasian or Asian descent tend to have rounder and softer hairlines. As opposed to both, North Americans and Australians have longer foreheads and higher hairlines.
5- Consult a Doctor
Choosing the hairline shape correctly is very important as hair transplantation is a permanent process. Hair transplant patients can choose their hairline based on the parameters mentioned above. Yet, if there is still uncertainty about hairline preference, patients can always consult their doctor. Doctors’ experience and understanding of the subject would be very helpful.
